What GLP-1s are

What GLP-1 medications
are, and how they work.

GLP-1 stands for glucagon-like peptide-1. It is a hormone your own gut already releases when you eat, and its job is to tell your body that food has arrived.

The hormone

Your body already makes it

Your own GLP-1 is released with a meal and breaks down within a few minutes. It has been part of normal digestion your whole life.

The medication

The medications make it last

These are longer-lasting versions of that same hormone, engineered to survive for days rather than minutes, so the signal keeps working between meals. They are not stimulants, and they are not doing anything your body does not already do.

What are they used for?

They began as a type 2 diabetes treatment and now cover considerably more. Approval is specific to individual products rather than to the class, so one medication carrying an approval does not mean another does. Each condition has its own page under approved uses, and some products are approved for adolescents from age 10 or 12 depending on the use.

How do they work?

Four things happen, and the mechanism is well described in the research.

  1. Insulin is released only when blood sugar rises The effect is glucose-dependent, meaning it happens when sugar is high and not when it is low. That is why these medications carry a low risk of hypoglycemia used on their own.
  2. Glucagon is suppressed Glucagon tells your liver to release stored sugar, and after a meal there is less need for it.
  3. The stomach empties more slowly Food stays put longer, so you feel full for longer and sugar enters the blood gradually instead of all at once.
  4. Appetite signaling changes in the brain This is where the weight effect comes from, and it is a separate mechanism from the blood sugar one.

The upside and the side effects share a cause

The same slowed stomach emptying that makes these medications work is what causes the most common side effects. The two are not separable, which is worth knowing before you start.

What is food noise?

The persistent, intrusive thinking about food that many people report going quiet on a GLP-1. It has its own page, with a simulator that moves you through a real weight-loss study so you can watch the hunger and fullness hormones respond.

How do the GLP-1 medications differ?

The names people ask about most are Ozempic, Wegovy, Mounjaro and Zepbound. Ozempic and Wegovy are the same drug, semaglutide, approved and dosed for different uses. Mounjaro and Zepbound are the same drug as each other, tirzepatide, in the same way. Tirzepatide acts on a second gut hormone receptor as well as the GLP-1 one, which is the main difference between the two families.

The rest of the differences are in dosing, in what each product is approved to treat, and in what your insurance will cover, which is often the deciding factor in practice. Compare them here.

The longer history

These medications did not appear in 2021. The line runs back to exendin-4, isolated from Gila monster venom in 1992, and the drugs that are still in trials now are the next stretch of the same line. The GLP-1 Museum is a catalogue of that record, with a page on what is in the pipeline and a side-by-side comparison that goes into more depth than the tool above.

The GLP-1 Museum is operated by GLP-1 Studio LLC, a for-profit company founded by the Collective's Executive Director. The Collective pays nothing for the link and receives nothing for it. How this works.

Where to go next

If you want to know what to expect when you start, read side effects and how to reduce them. If you are trying to work out whether these medications apply to a condition you have, the approved uses pages cover each one. If you are struggling to get a prescription covered, self advocacy and prescription assistance are the practical routes.
